What is CAC-DRS?

The Agatston score quantifies calcified coronary plaque on a non-contrast cardiac CT. A score of 0 indicates no detectable calcified plaque, while higher scores reflect greater calcified atherosclerotic burden and higher cardiovascular risk.

How are CAC-DRS categories defined?

CAC-DRS groups total Agatston scores into four categories: 0 (score 0), 1 (1–99), 2 (100–299), and 3 (≥300). A modifier indicates the number of vessels involved, and these categories map to escalating cardiovascular risk and management.

What does a calcium score of zero mean?

A score of zero (CAC-DRS 0) reflects no detectable calcified coronary plaque and a low short-term risk of cardiovascular events in most patients. It can support deferring or de-intensifying statin therapy in selected intermediate-risk individuals, per current guidance.
